Use this template to prepare an Employee Statutory Declaration for employees of a company which is party to a syndicated mortgage transaction in Alberta, to allow the employees to purchase mortgage units.

• The employee must provide the issuer with the Statutory Declaration in compliance with Securities Act regulations.
