Minnesota Mechanics Lien Statement and Affidavit of Service
File notice of a lien for labor, materials or machinery supplied for a construction project with this Minnesota Mechanic’s Lien Statement, which comes bundled with an Affidavit of Service of Mechanic’s Lien Statement form.
Contractors, subcontractors, laborers and suppliers can use this form to claim their lien. The Lien Statement must be served on the owner, the owner’s agent, or the person who authorized the work within 120 days after the last of the work has been performed or the last supplies have been provided.
