Order the probation office to conduct a presentence investigation and report on a defendant with this free US District Court form.

• The probation officer must provide the report to legal counsel for both sides at least 35 days before sentencing.
• The attorneys then have 14 days to file any objections.
• At least 7 days before sentencing, the probation officer must submit the final report to the court.
